Submission
Submit with your procedure; a downloaded form is not an application.
Before you apply
Find “Application Form (Report Form)” on the official list and choose its PDF, HWP or DOC icon.
Select the procedure you are applying for and match names and passport details exactly.
Ask the responsible office for the additional documents, fee and submission route for your status.
